In order to ELIMINATE hospital re-admissions, we have begun a new program! With the approval of the physician, clinical team and insurance, a CONCIERGE CARE MEMBER is assigned immediately. This team member is carefully chosen and placed in the patient’s home to ensure constant range of motion (ROM) assistance and the completion of all medical goals.
Simply have your doctor include the following on the RX:
Eval and treat for HH with CNA 4w8 for ROM
And we will take care of the rest!
